The etiology of this condition remains unresolved. We describe a unique patient who presented with a cluster headache syndrome concurrent with new-onset pseudoaneurysm within the cavernous sinus. Our review and analysis of this patient confirms current theories which describe the role of the cavernous sinus as a locus of pathology in cluster headache.
